If you are unable to complete the form online, you are welcome to come, but

please bring something in writing with your name, address, and animal information,

including pet name, age, color, breed, and sex.

Humans must wear masks and practice social distancing.

Pets must be on leashes or contained in pet carriers. 

TOWN LICENSING WILL BE AVAILABLE

AT THIS CLINIC AS WELL
